A low number may cause fear; a high one may bring relief. Neither reaction tells the whole story.

Anti-Müllerian hormone (AMH) is a useful test in the right situation. But it is not an egg count, a fertility score or a personal deadline for having a child. It helps doctors estimate how the ovaries may respond to stimulation; on its own, it cannot predict whether you will have a baby. [1]

What does AMH actually measure?

AMH is produced by small, developing follicles in the ovaries. Its blood level is an indirect marker of the quantity of ovarian reserve, not egg quality. It does not count all the eggs remaining in the ovaries. [1,2]

AMH generally falls with reproductive age, but results differ substantially between women of the same age. A lower-than-average result is not the same as infertility. [1,2]

If my AMH is low, can I get pregnant naturally?

Yes, natural pregnancy may still be possible. The important question is how much a single AMH result can tell us about your individual chances. The answer is: not very much.

In a 2017 study of 750 women aged 30–44 without known infertility, the estimated chance of conception after six cycles was 65% among those with low AMH and 62% among those with normal AMH. The difference was not statistically significant. Other studies, including a meta-analysis of 11 studies involving 4,388 women, found that AMH was a poor stand-alone predictor of spontaneous pregnancy. The studies included women with different reproductive histories and medical conditions, so their average results cannot predict an individual’s chances. A follow-up study found no clear reduction in future births among women with diminished ovarian-reserve markers. [3–6]

A larger study published in 2024, involving 3,150 women, did find a modest association: women with AMH below 1 ng/mL had a somewhat lower chance of conception per cycle. For example, the estimated chance of conceiving in the fourth cycle was 11.2%, compared with 14.3% among women with normal AMH. These are chances for that cycle, not the total chance after four cycles. All participants had purchased a commercial hormone test, which limits how widely the findings can be applied. The study was funded by Modern Health, which operates the hormone-testing company Modern Fertility. [7]

The findings are not perfectly consistent, but AMH alone cannot tell an individual woman whether or when she will conceive. Conception and live birth are also different outcomes: a 2023 review found too little evidence to use AMH to predict live birth among women trying to conceive without fertility treatment. [1,5,7,22]

What does the result leave out?

Pregnancy also depends on age-related egg quality, ovulation, the fallopian tubes, sperm, the uterus and other health factors. An AMH blood test does not assess any of these. [1,8]

Age matters particularly when planning a future pregnancy. A normal AMH at 39 does not make the ovaries biologically younger, just as a low AMH at 29 does not prove infertility. Current ACOG guidance encourages thoughtful discussion of reproductive plans and age-related fertility changes—not promises based on a single blood test. [8,9]

How should I interpret my number?

Check the units: laboratories may report AMH in ng/mL or pmol/L (1 ng/mL is approximately 7.14 pmol/L). Compare results using the laboratory’s assay and age-appropriate context. There is no universal cut-off that separates “fertile” from “infertile”. [1,2]

Tell your clinician if you use hormonal contraception. In a study of 1,643 African American women aged 23–35, current users had AMH levels about 25% lower on average than women who had never used hormonal contraception. This was a comparison between groups, not a prediction for every woman; the size of the difference may vary with contraceptive method and population. AMH can rise after stopping contraception: in a prospective study of combined-pill users, levels generally stabilized within about two months. A 2026 brief clinical commentary notes that suppression may last longer in some cases, but this is not established for every method. Do not stop contraception just to change your AMH number; ask whether retesting would affect a clinical decision. [10–12]

Mention any history of ovarian surgery, endometriosis, cancer treatment or irregular periods. An isolated result should always be interpreted alongside your circumstances. [1]

What if my AMH is high?

A high AMH does not guarantee a pregnancy. It can occur in polycystic ovary syndrome (PCOS), where ovulation may be irregular. In adults, AMH may be used instead of ultrasound to identify polycystic ovarian morphology as part of the diagnostic assessment. AMH alone does not diagnose PCOS and is not recommended for this purpose in adolescents. [13]

When is AMH testing genuinely useful?

Before IVF or another controlled ovarian-stimulation cycle, AMH or the ultrasound antral follicle count can help estimate the response and likely number of eggs retrieved. Some studies have found an association between AMH and live-birth outcomes after IVF, but the test is not reliable enough to predict an individual person’s chance of having a baby. No AMH threshold has been identified below which a live birth cannot occur, and very low AMH alone should not exclude someone from IVF. [1,14,22]

Before egg freezing, AMH may help estimate how many eggs a stimulation cycle could yield. The chance of a future baby from frozen eggs depends on other factors, particularly age at freezing; AMH is not a personal guarantee. [1,15]

With an ovarian endometrioma, AMH may help assess the potential impact of ovarian disease or surgery. Surgery can reduce ovarian-reserve markers; ESHRE does not recommend routinely removing an endometrioma before assisted reproduction solely to improve live-birth rates. A 2026 analysis found differences in postoperative AMH across surgical methods, but did not establish better future pregnancy or live-birth outcomes from preserving the number. [16,17]

When premature ovarian insufficiency is suspected, AMH may provide supporting information if the diagnosis is uncertain. It is not the primary diagnostic test: menstrual changes and FSH results are central to assessment. [18]

Before cancer treatment or another medical treatment that may damage fertility, AMH may be part of an individual assessment. But fertility-preservation counseling and referral should happen promptly when appropriate; they should not be delayed while waiting for an AMH result. [19,23]

Can AMH predict menopause?

Not reliably for an individual woman. A systematic review of 41 studies including 28,858 women found that lower AMH was associated with earlier menopause at a population level, but an individual prediction remained too imprecise to serve as a personal countdown—especially below age 40. [20]

When should I seek fertility advice?

If you are trying to conceive, assessment is generally advised after 12 months without pregnancy when you are younger than 35, or after six months between ages 36 and 40. At exactly 35, professional recommendations differ: ASRM advises assessment after six months, while ACOG advises after 12 months, so discuss the timing with your clinician. Over 40, seek assessment promptly. Do not wait for these time limits if you have irregular or absent periods, suspected endometriosis or tubal disease, a known sperm problem, previous ovarian treatment, or another clear concern. [9,21]

Three situations, three different conversations

“I am 29, my cycles are regular and my AMH is low.” This is not a diagnosis of infertility. Discuss your plans and any relevant history rather than drawing a conclusion from the result alone.

“I am 39, my AMH is normal and I want to wait several years.” The test cannot promise that fertility will remain unchanged. Talk about age, timing and realistic options.

“I have an endometrioma or need chemotherapy.” Here, AMH may contribute to a decision—but protecting health, considering treatment risks and discussing fertility-preservation options are more important than chasing a laboratory number.

The question worth asking

Before ordering or repeating AMH, ask: “What decision will this result help us make?” If it helps plan stimulation or clarify a specific clinical issue, the answer may be useful. If the goal is to learn exactly how many fertile years remain, the test cannot deliver that answer.

A good conversation about fertility considers the whole person: her age, health, wishes, circumstances and available choices. One blood-test result should never replace that conversation.

Medical note: This article is for general education and cannot replace individual advice from a qualified healthcare professional.

Scientific references

1.
